After impact·Gonstead method
Whiplash
Symptoms often peak in the days after the impact, not in the car park. Waiting for it to settle on its own is how a mild sprain becomes a long neck problem.
Understanding it
What is whiplash?
The head is thrown one way and rebounds the other. Discs, small joints, ligaments, and muscles in the neck are taken past their usual range. Rear-end crashes are the classic cause. Sport, falls, and a direct blow do the same thing.
How smashed the bumper looks does not tell you how hard the neck was loaded. A slow impact can still sprain joints. An early, specific check looks for the segments that stopped moving before the pattern hardens.
This page explains how a visit is organised. It is not a diagnosis. Get urgent medical care for sudden weakness, loss of bladder or bowel control, fever with spine pain, or pain after a major fall or crash.
Root patterns
How the neck gets whipped
The direction, the speed, and whether you saw it coming all change which tissues are hurt.
A rear impact
The seat pushes the body forward while the head lags, then the head snaps forward. That is extension followed by flexion.
Falls and blows
Landing on the head or shoulder, or being struck, can load the neck the same way without a car.
Contact sport
Tackles, throws, and repeated smaller impacts add up into the same joint and ligament picture.
Bikes, slopes, rides
A fall or a sudden change of direction can match a low-speed crash.
With a concussion
A hit hard enough to concuss almost always strains the neck too. Headache, dizziness, and fog can belong to both, so both need a look.
The delay
Adrenaline hides pain for hours or a couple of days. Feeling fine at the scene does not mean the joints were spared.
Progression
A simple grading
This follows the usual four grades. The grade decides whether hands-on care is reasonable.
01Grade I
Pain and stiffness only
Tender, limited, no arm symptoms and nothing alarming on a film. Gentle, specific work is reasonable once serious injury is excluded.
02Grade II
The exam shows a joint problem
Motion is measurably down and spots are tender. This is the common post-crash visit.
03Grade III
The arm is involved
Numbness, tingling, or weakness means a nerve or a disc may be in the picture. Films and a neuro check come before any contact.
04Grade IV
A fracture or a dislocation
That is emergency care. Spinal contact is not appropriate until a medical team has cleared the bone.
A check in the first week, even when the pain is mild, is kinder than waiting for a chronic pattern. Worsening weakness, or bowel and bladder change, is urgent care first.
Recognition
What shows up after the impact
Symptoms that arrive or worsen two or three days later are the usual inflammatory timing, not proof that it was minor.
A neck that will not turn
All directions are smaller, and the second and third days are worse than the first.
Headache at the skull base
Upper neck joints and muscles are the usual source.
Arm tingling
One or both arms. That raises the grade.
Dizziness or visual oddness
The upper neck helps tell you where your head is. A sprain can disturb that.
Fog and irritability
Concentration and mood can shift in a moderate injury. A concussion still needs its own medical check.
“I felt all right leaving the workshop after a low-speed bump. Two days later the headache sat at the base of the skull. Puchong checked the upper neck before anything was contacted.”

Questions
Common questions
Irritation and swelling build over hours or days. A small crash is still worth a check if the neck, head, or arm is involved.
