Slipped Disc
Pain that shoots down a leg has a level. The visit tries to find that level before anyone talks about a general crack.
Understanding it
What is a slipped disc?
A disc sits between two vertebrae. Its softer centre can push through a split in the tougher outer ring and bother a nearby nerve. People feel pain, numbness, or weakness into an arm or a leg, depending on the level.
The low back, especially the lower two discs, is the common place. The neck is the other. It is a frequent reason adults in mid-life cannot sit or walk comfortably. Many cases change without an operation. That is not a promise for every scan.
This page explains how a visit is organised. It is not a diagnosis. Get urgent medical care for sudden weakness, loss of bladder or bowel control, fever with spine pain, or pain after a major fall or crash.
Root patterns
How a disc usually gets there
A herniation is rarely a single unlucky morning. The outer ring has often been tired for a long time.
Discs lose water with age
A drier disc cushions less and tears more easily, even under ordinary load.
A rounded lift
Lifting with a bent back instead of the legs shears the low-back discs. One bad lift can finish a disc that was already weak.
Years of sitting
Sitting raises pressure inside the disc compared with standing. A slumped desk posture speeds wear at the lower levels.
Extra body weight
More load through the low back raises the chance of a lumbar herniation. It is one factor among others, not a verdict.
Smoking and poor recovery
Discs have a thin blood supply. Smoking and a thin recovery diet make small tears slower to settle.
A family pattern
Some people inherit discs that tire earlier. If disc trouble runs in the family, an earlier check is worth more than waiting for the leg pain.
Progression
Four stages people are given
The name on a report matters less than whether the outer wall is still intact and whether a nerve is compressed.
01Early
The disc is drying out
Shock absorption drops. There may be no bulge yet, only stiffness after long sitting.
02Bulge
The wall bows
The centre pushes outward and the shape changes, but the outer ring still holds. Bending and lifting bring intermittent pain.
03Extrusion
Material has come through
Disc material breaks the outer ring and can press a nerve root. Radiating pain, numbness, and weakness are common. This is what most people mean by a slipped disc.
04Fragment
A piece has separated
Material can move in the canal. Weakness, reflex loss, and in rare cases bowel or bladder change need urgent medical care, not a routine visit.
Contacts are aimed at stages where the segment can still be unloaded. A free fragment with worsening weakness is a medical emergency first.
Recognition
Patterns that fit a disc
If several of these sit together, a precise exam is the next step, often with films or a scan.
Sharp local back pain
Pain parked at one level rather than a vague whole-back ache.
A line of pain down one leg
It follows a nerve path and is usually stronger on one side.
Numb toes or foot
Sensation drops in a strip of the foot or the toes.
Walking feels unsafe
The leg gives or the stride shortens because the muscles are not firing evenly.
Weakness
Pushing off, climbing stairs, or lifting the foot gets harder.
“Sciatica from a disc had me standing up in meetings. The Mont Kiara plan was specific about the level. I was walking a normal block again without daily tablets.”

Questions
Common questions
Many do, including people who arrive limping. Some do not. The exam, and sometimes imaging, decides whether hands-on care is reasonable or whether you need a surgeon or a hospital the same day.
