Shoulder Pain
The shoulder is the most mobile joint you have, so it is easy to overload. The neck and mid-back often keep the pain going after the shoulder itself has been treated.
Understanding it
What counts as shoulder pain here?
Pain can come from the ball-and-socket joint, the rotator cuff, a bursa, the joint at the end of the collarbone, or from nerves in the neck. Local treatment fails when the driver is one of the others.
The exam looks at neck alignment, how the mid-back moves, and how the shoulder blade sits, then decides which of those is feeding the pain.
This page explains how a visit is organised. It is not a diagnosis. Get urgent medical care for sudden weakness, loss of bladder or bowel control, fever with spine pain, or pain after a major fall or crash.
Root patterns
Where shoulder pain starts
The neck’s share is the part most often skipped.
Rotator cuff strain
The small muscles that keep the ball centred can be overloaded or partly torn. The ache is deep and lifting feels weak.
Impingement
Tendon and bursa get pinched as the arm lifts. A painful arc in the middle of the lift is the usual clue.
A stiff capsule
The capsule can thicken and steal motion, especially outward rotation, sometimes with no clear injury. People call this a frozen shoulder.
A nerve from the neck
Irritation around the mid-neck can send pain into the deltoid and upper arm and look exactly like a shoulder injury.
A rounded mid-back
When the shoulder blade tips forward, the space above the tendon narrows. The cuff then works overtime even on easy days.
Overhead work
Swimming, throwing, pressing, and work above shoulder height fray the cuff and the biceps tendon a little at a time.
Progression
How it tends to worsen
Early irritation is much easier to change than a tear or a frozen capsule.
01Early
Only at the end of the movement
Strength is still there. Posture, mid-back motion, and a neck segment are often enough to settle it.
02Painful arc
A band of the lift hurts
Night pain starts, especially lying on that side. The tendon is beginning to change, not only inflame.
03Weak
Daily reach is hard
Reaching behind the back or overhead is painful or weak. A frozen shoulder shows up here as a large loss of outward rotation.
04Structural
A full tear or a locked shoulder
A large cuff tear needs a surgical opinion. A frozen shoulder can thaw, but it is slow. The neck and mid-back still matter beside either path.
Shoulders that never settle are often being driven by the neck or the mid-back. Treating only the joint leaves that driver in place.
Recognition
Signs to bring to the visit
Pain past a few weeks, pain that wakes you, or tingling down the arm deserves a look past the shoulder.
It hurts to lift the arm
Out to the side or forward, often in the middle of the arc.
You cannot sleep on it
Night pain is common once the tendon is involved.
Overhead feels weak
A shelf, a seatbelt, or a shirt behind the back is the test.
Rotation is small
Combing hair or tucking in a shirt has shrunk.
The arm tingles
That points back toward the neck.
“I had stopped sleeping on my left side. Puchong checked the neck as well as the shoulder, and the night pain was the part that changed first.”

Questions
Common questions
Yes. Nerve or joint trouble in the neck can refer straight into the shoulder. Both are checked.
