Knee Pain
The knee is a hinge between the hip and the foot. Pain there is often a loading problem arriving from above or below.
Understanding it
What is knee pain in this clinic?
The knee can hurt because of the joint surfaces, the meniscus, a ligament, or a tendon. It also hurts when the hip rolls in, the foot collapses, or the pelvis sits unevenly, so every step loads one side of the knee.
Rest, a brace, or an injection at the knee can quiet a month. If the load from the hip or pelvis is unchanged, the same activity brings the pain back. The exam maps the low back, pelvis, hip, knee, and foot before deciding where the contact belongs.
This page explains how a visit is organised. It is not a diagnosis. Get urgent medical care for sudden weakness, loss of bladder or bowel control, fever with spine pain, or pain after a major fall or crash.
Root patterns
Common drivers
In active adults the knee is often the messenger, not the only problem.
Kneecap tracking
The kneecap drifts outward when the thigh muscles are uneven or the hip is weak. Runners and cyclists know this ache well.
IT band irritation
Pain on the outer knee is frequently a hip that drops on that side, not a knee that needs to be rubbed in isolation.
Meniscus irritation
A twist or a deep squat can fray or tear the cushion between the thigh and shin bones. Some tears need a surgeon. Many irritations do not.
Pelvis and hip
A tipped pelvis can make one leg behave as if it is longer, so one knee takes more compression on every step.
Tendon overload
Jumping and repeated squats load the tendon under the kneecap. Stairs downward are the usual complaint.
Early joint wear
Thinning cartilage, often on the inner knee, gives morning stiffness and an ache after activity. Better alignment does not regrow cartilage. It can slow uneven wear.
Progression
How knee pain tends to build
Earlier correction of the load is kinder than waiting for the joint surface to change.
01Early
Only after the activity
A long run, a match, or a squat day. It settles within hours. The joint is annoyed, not ruined.
02Repeating
You start editing the activity
Shorter runs, avoided stairs, an ache that lingers after exercise. The load problem is now constant.
03Daily
Walking and standing join in
Swelling may show. Meniscus and cartilage changes are harder to undo the longer this stage runs.
04Structural
A tear or bone-on-bone wear
A complete tear or advanced wear may need surgery. Afterward, the same hip and pelvis pattern can stress the repair if it is ignored.
A large share of stubborn knee pain is a hip or pelvis pattern showing up at the knee. Changing that load is the point of the visit.
Recognition
Patterns worth a check
Pain that returns with the same activity, or that comes with swelling, is a load signal.
Stairs
A sharp or aching pain on the way down.
Running
It builds during the run or arrives the next morning.
Swelling or warmth
The joint looks puffy or feels hot after use.
Catching or a click
The knee locks for a moment or clicks in the same spot.
A stiff start to the day
The first minutes out of bed are the worst, then it eases.
“I was told to stop running. The check in Ampang was mostly about a weak hip and an uneven pelvis. The knee was never the only thing on the table, and the 10k came back.”

Questions
Common questions
Yes. Pelvis, hip, low back, foot, and posture all change how weight crosses the knee. The exam looks at that chain.
