Hip Pain

Pain at the hip can be the joint, the muscles, or a nerve from the low back. Naming the source is the whole first visit.

Understanding it

What is hip pain in this clinic?

The hip is where the spine, through the pelvis, meets the leg. Groin pain, side-of-hip pain, and buttock pain are not the same tissue. A lumbar nerve, a sacroiliac joint, a bursa, or the hip joint itself can all be the one talking.

The exam checks lumbar alignment, whether the pelvis is level, and how the hip actually rotates. Many people sent for a hip injection find the driver was the pelvis or the low back.

This page explains how a visit is organised. It is not a diagnosis. Get urgent medical care for sudden weakness, loss of bladder or bowel control, fever with spine pain, or pain after a major fall or crash.

Root patterns

Sources that feel like the hip

Treating the wrong one is the usual reason nothing changes.

Joint wear

Cartilage loss gives a deep groin ache, stiffness after sitting, and less inward rotation. It is more common after about fifty.

The ball catching the socket

Extra bone can pinch at the front of the hip in younger active adults when the hip bends. Groin pain on a deep squat is typical.

The sacroiliac joint

This joint between the sacrum and the pelvis can ache in the buttock and be mistaken for the hip or a disc.

A lumbar nerve

Irritation higher in the low back can refer into the groin and front of the thigh and imitate a hip joint.

A bursa on the side

The point of the hip is tender to lie on. Runners and people after pregnancy know this one.

Sitting and sleepy glutes

Long sitting shortens the front of the hip and switches the glutes off. The pelvis tips and both the hip and the low back complain.

Progression

How it builds

Pelvic and lumbar alignment change how fast a hip wears. They are part of every stage.

  1. 01Early

    Stiff after sitting

A walk or a match leaves an ache. Full motion is still there. Pelvic and lumbar work often stops it here.

  2. 02Activity

    Stairs and cars join in

Getting out of a car or climbing starts the groin or the side of the hip. The limp begins, and the knee and back start sharing the load.

  3. 03Daily

    Rest no longer clears it

Night ache and a visible limp. The other side starts to overload. Walking distance shrinks.

  4. 04Joint failure

    Bone on bone

A replacement is sometimes the right medical step. The lumbar and pelvic pattern that came before it still needs a plan, including after surgery.

A lot of hip pain is a pelvis or a lumbar segment wearing a hip’s clothes. Telling those apart changes the whole plan.

Recognition

What to notice

A limp, a shorter walk, or pain that wakes you is enough reason for a full check, not only a hip film.

  • Deep groin ache

The usual place when the joint itself is involved.

  • Stiff on the first stand

The chair or the bed is worse than the tenth step.

  • Socks and crossing legs

Inward rotation shrinks early when the joint is wearing.

  • Tender on the side

Lying on that side hurts. Think bursa, not only the joint.

  • A shorter step

You shift off the sore hip without deciding to.

“The ache sat in the buttock, so I assumed the hip joint. Mont Kiara found the sacroiliac side was the one that would not move. Stairs got easier once that was the focus.”

Questions

Common questions

Groin and side-of-hip pain lean toward the joint. Buttock and leg pain can be the spine. The exam separates them.
