Carpal Tunnel
Night tingling in the thumb and fingers is a median nerve under pressure. The squeeze is not always at the wrist.
Understanding it
What is carpal tunnel syndrome?
The median nerve passes through a tight tunnel at the wrist. When pressure there rises, the thumb, index, and middle finger tingle or go numb, classically at night.
The same nerve can already be irritated in the neck, around the shoulder, or in the forearm. A nerve that is bothered higher up becomes symptomatic at a lower wrist pressure. That is why a wrist-only operation sometimes disappoints. The visit follows the nerve from the neck to the hand.
This page explains how a visit is organised. It is not a diagnosis. Get urgent medical care for sudden weakness, loss of bladder or bowel control, fever with spine pain, or pain after a major fall or crash.
Root patterns
Where the nerve gets crowded
Missing a squeeze higher up is a common reason the wrist treatment does not hold.
The desk wrist
Hours with the wrist bent up or down raises pressure in the tunnel.
The lower neck
A disc or joint around the lower neck can irritate the roots that become the median nerve, so the wrist symptoms start sooner.
The shoulder girdle
Muscle at the side of the neck or under the chest can compress the nerve before it reaches the wrist and copy the same finger pattern.
Fluid and medical conditions
Pregnancy, thyroid disease, diabetes, and inflammatory arthritis can raise pressure in the tunnel. Those still need their own medical care.
Vibration
Drills, grinders, and sanders injure the nerve a little at a time.
A tight tunnel
Some wrists are narrow. A cyst, or sleeping with the wrist bent, makes the space smaller still.
Progression
From night tingling to a weak thumb
Each stage is a narrower window for care that does not involve surgery.
01Night
It wakes you, then shaking clears it
Thumb, index, and middle finger. Daytime can still feel normal. Nerve tests may still be clear.
02Day
Driving and typing join in
The forearm may ache. Grip endurance drops.
03Constant
The numbness stays
Buttons and small objects get clumsy. The muscle at the base of the thumb can start to flatten. Nerve tests usually confirm slowing.
04Wasting
The thumb mound shrinks
Sensation may not fully return if this is left. Release surgery is often discussed. The neck and forearm still need a look so the same pattern does not continue after the operation.
Early night symptoms often ease when the neck and the wrist are both addressed. A flattened thumb muscle should not wait.
Recognition
The pattern people recognise
Waking to shake a numb hand, for more than a month, means the nerve is under sustained pressure.
Thumb-side tingling
Thumb, index, middle, and half the ring finger. The little finger is usually spared.
Night waking
You shake the hand to bring it back. That is the early signature.
A clumsy grip
Jars, keys, and dropped mugs.
Forearm ache
It can track toward the elbow, which is a hint to look higher than the wrist.
A flatter thumb mound
Late. Have it seen promptly.
“The tingling was worse on the drive than at the keyboard. Ampang followed the nerve up into the lower neck instead of stopping at the wrist, and the night waking eased over the next month.”

Common questions
The classic map is the thumb, index, middle, and half the ring finger. Other nerves and the neck can copy it, so the pathway is tested rather than assumed.
